3/12/2023 EnchantaVaughn Approved 1.Behavior: Client was active and engaged when QMHP-C entered the home, as evidenced by her smiling and interacting well with her mother. Client made continuous eye contact during the session. Client participated in the activity without getting frustrated and giving up easily 2. Behavior: Client was more mindful and seemed to enjoy today’s session after talking about her triggers and failures with her QMHP-C. Client continued to struggle with managing her anger as she was getting frustrated with her mother when talking about her behaviors in the home. 1.Intervention: QMHP-C encouraged Client to make good choices and to utilize self-calming techniques (i.e. counting and deep breathing) and processed with her that this behavior is acceptable and discussed actions and consequences. QMHP-C will teach Client necessary coping skills to decrease impulses. QMHP-C will brainstorm strategies with Client and encourage self-monitoring. QMHP-C will provide mediation and relaxation techniques. QMHP-C will collaborate with parent/close family member regarding behaviors and performance. QMHP-C intervened in an effort to encourage Client to make the right decision. QMHP-C explained the importance of being on task and refraining from talking in order to complete her assigned task. QMHP-C addressed Client’s negative behavior. 2. Intervention: QMHP-C redirected Client’s negative behavior. QMHP-C ensured Client was feeling okay as she appeared to be distant. QMHP-C removed Client from the group to engage in one on one. QMHP-C allowed Client to individually process what occurred. QMHP-C validated Client’s feelings but explained the importance of utilizing her coping skills. QMHP-C stated “You have to learn to ignore others and not be so easily triggered.” QMHP-C reviewed Client’s coping skills. QMHP-C isolated Client from distractions to decrease disruptive behaviors and to reduce excessive talking. QMHP-C explored Client’s capability to cope with her stressors. 1. Response: Client verbalized positive feelings about achieving goals and objectives. Client communicated that she wants to work independently. Client displayed good participation in various settings and was responsive to all instructions, interacting well with her peers. Client utilized coping skills and impulses started decreasing. Client was observed strategizing a plan to better himself by writing down her thoughts and feelings and exploring her creative side with arts and crafts. Response: Client was taught to take deep breaths and count to 10. Client was reported displaying age appropriate behaviors and was productive and studious in the home today. Client thanked QMHP-C. Client was acceptive of the interventions provided by QMHP-C. Client understood the importance of listening and following directions and have reduced time spent outside to learn more about her wrongdoing and shows initiative to correct them. Client utilized relaxation strategies. Client was seen decreasing anger and frustration.
